SECTION 310:281-5-2. Issuance of permit

(a) Any person(s) wishing to operate or cause to be operated a diagnostic x-ray system shall make written application for a permit on forms provided by the Oklahoma State Department of Health. The applicant(s) shall be the owner or lessee of the system(s). Such application shall include the name and address of each applicant, the location and type of diagnostic x-ray facility, name of service agent, where applicable, and the signature of each applicant or his authorized agent.
(b) No permit will be issued except upon successful registration of the diagnostic x-ray system with the Department, compliance with applicable rules, and payment of the appropriate permit fee.
