SECTION 310:590-3-3. Podiatric health care provider services

- When determined as medically necessary for the prevention of lower extremity complications and upon referral by a physician, all individual or group health insurance defined by statute shall reimburse/cover as defined by statute the following, but not limited to, podiatric health care provider services:(1) fitting of therapeutic molded or depth-inlay shoes, replacement inserts, preventive devices, and shoe modifications;(2) callus and nail trimming;(3) complex evaluation of sensory loss; and(4) treatment of ulcer with total contact casting.
