- (a) A medical record shall be maintained for every patient cared for in the ambulatory surgical center. The medical records shall be filed for easy access by the medical staff or representatives of the licensing agency.(b) The medical record shall contain, as a minimum, the following:(1) Patient identification.(2) Patient history, physical examination, chief complaint, copies of any laboratory, X-ray, or consultation reports.(3) Description of surgical procedures performed, observations, anesthesia records and disposition of the patient.
