  • (a)   Transcript of hearing. Upon written request reasonably made by a person affected by the hearing, and at such person's expense, a full stenographic record of the proceedings shall be made by a competent court reporter. When such transcription shall be made a part of the record of the Insurance Commissioner, any other person having a direct interest therein shall be furnished with a copy of such stenographic record at his expense.
    (b)   Record contents. The record in any hearing shall include the following:
    (1)   All pleadings, motions and intermediate rulings;
    (2)   All evidence received or considered including a statement of matters officially noted;
    (3)   Questions or offers of proof, objections and rulings thereon;
    (4)   Proposed findings and exceptions;
    (5)   The decision, opinion, report or order of the Insurance Commissioner.
