SECTION 450:18-7-23. Biopsychsocial assessment

(a)   All programs shall complete a biopsychsocial assessment using the Addiction Severity Index (ASI) which gathers sufficient information to assist the consumer in developing an individualized service plan. The assessment must also list the client's past and current psychiatric medications. The assessment must be completed by a Licensed Behavioral Health Professional or Licensure Candidate.

(b)   Compliance with 450:18-7-23 may be determined by a review of the following:

(1)   Policy and procedures;

(2)   Biopsychsocial assessment instruments;

(3)   Consumer records;

(4)   Case management assessments;

(5)   Interviews with staff and consumers; and

(6)   Other facility documentation.
