SECTION 450:21-1-7.6. Participant records, basic requirements

- (a) Participant records shall be developed and maintained to ensure that all appropriate individuals have access to relevant course and other information regarding the participant. The record shall communicate information in a manner that is organized, clear, complete, current and legible.(b) Entries in participant records shall be legible, signed with first name or initial, last name, and dated by the person making the entry.(c) The participant shall be identified by name on each sheet in the participant record.(d) A signed consent in a form designated by the Commissioner or designee shall be part of the case record for any person admitted to an ADSAC course.
