SECTION 450:21-1-7.7. Participant record storage, retention and disposition

- (1) Limit access to participant records to persons on a need to know basis;(2) Require participant records be stored under lock and key;(3) With regard to closed participant records, require:(A) Confidential storage under lock and key;(B) Record disposition and destruction under confidential conditions; and(C) Maintain written documentation to be available for participants for a minimum of six (6) years after completion of all course requirements. Written documentation shall include, but not be limited to:(i) completed pre and post tests;(ii) notes, if existing;(iii) proof of completion of course; and(iv) enrollment form.(b) EXCEPTION: With regard to 450:18-7-4(a) (3) (B), facilities operated by ODMHSAS shall comply with the provisions of the Records Disposition Schedule for said facility as approved by the Oklahoma Archives and Records Commission [67 O.S. § 305 and OAC 60:1-1-2].
