SECTION 595:11-9-9. Fee schedule and payment

Latest version.

(a)   The fee schedule shall be:
(1)   The fee for an original application for subscription by an employer shall be Fifty Dollars ($50.00).
(2)   The fee for a renewal subscription of an employer shall be Fifty Dollars ($50.00).
(3)   The fee for an original enrollment of a driver by an employer shall be Five Dollars ($5.00) plus the fee for a Motor Vehicle Report as prescribed in 47 O.S. § 6-117.
(4)   The fee for a renewal enrollment of a driver by an employer shall be Five Dollars ($5.00) plus the fee for a Motor Vehicle Report as prescribed in 47 O.S. § 6-117.
(b)   Companies will be invoiced for payment to the Department of Public Safety. Payment is due upon receipt of invoice.
(c)   No fee shall be subject to refund or to prorating.
