SECTION 595:65-1-2. Collision report forms

Latest version.

(a)   All motor vehicle accidents and collisions shall be reported by the investigating law enforcement occicer on the "Official Oklahoma Traffic Cokkision Report". The latest version of blank forms are availabe from the Department of Public Safety:
(1)   Material Management Division , PO Box 11415, Oklahoma City, OK 73136, or
(2)   on it website: http://www.dps.state.ok.us/otcr/
(b)   All completed forms shall be submitted by the investigating law enforcement agency to the Department of Public Safety, Records Management Division, PO Box 11415, Oklahoma City, OK 73136.
