SECTION 655:20-1-2. Availability of forms and instructions

(a)   The following forms and instructions are issued by the agency for public use.

(1)   Certificate of incorporation and instructions (profit, not for profit and professional)

(2)   Amended certificate of incorporation (profit, not for profit and professional)

(3)   Certificate of dissolution (profit and not for profit)

(4)   Revival and extension (profit and not for profit)

(5)   Restated certificate of incorporation (profit and not for profit)

(6)   Certificate of qualification and instructions (foreign corporation)

(7)   Foreign corporation fictitious name

(8)   Amended certificate of qualification

(9)   Certificate of withdrawal

(10)   Certificate of merger (domestic and foreign corporation)

(11)   Certificate of limited partnership and instructions

(12)   Amended certificate of limited partnership

(13)   Restated certificate of limited partnership

(14)   Cancellation of limited partnership

(15)   Foreign limited partnership registration and instructions
