SECTION 725:15-19-5. Form of application
Latest version.
- No standardized application form is necessary to apply for funds under this program. A letter or other written request for funds shall be submitted to the Department with the following information:(1) Information which must be included is:(A) Name and address of applicant,(B) Amount of money requested,(C) A narrative description of the purpose for which the funding is requested.(2) Supplemental information which is not required, but is of benefit in determining the relative merit of a proposal is as follows:(A) A statement as to whether the project will provide permanent improvements, promotions, a one-time special event or a periodic event,(B) An estimate of the time period over which benefits will be received due to this project and the number of people who will be benefited,(C) A statement as to whether program funds are being requested as a grant or if other funds, services or materials are being provided from other sources for this project,(D) A statement as to the public support which exists for this project,(E) A statement as to whether funds will be necessary in subsequent years to continue the project,(F) Provide a copy of the site plan for the project, if applicable.
