SECTION 800:1-15-4. Administrative requirements

- Course report forms and rosters of students shall be submitted in writing to the Program Coordinator by the Game Warden or volunteer instructor of the course.(1) The course report form shall include the following information: location, hours, dates, city, county and address of the course.(2) The form shall also include the number of students attending the course, the number of students who failed and the number who were certified.(3) Course report forms are to be dated and signed by the instructor(s) verifying that the course was conducted in compliance with program requirements.(4) Course report forms and rosters are to be submitted to the Program Coordinator office by the Game Warden or volunteer instructor of the course within 10 working days of completion of a course.(5) An estimate of the ethnic background and gender of the students receiving certification shall also be included.
