Editor's Note: 1This emergency action expired without being superseded by a permanent action. Upon expiration of an amendatory action revoking and reenacting an Appendix, the last prior permanent text is reinstated. Therefore, on 7-15-05 (after the 7-14-05 expiration of the emergency action), the text of Appendix A reverted back to the permanent text effective prior to the 6-4-04 enactment of the emergency action, as was last published in the 2001 OAC Edition, and remained as such until revoked and reenacted by permanent action on 7-13-06.
