- (a) Monthly reports. No later than the 10th day of each month, each facility shall submit a report to DEQ that includes the following information for the previous month:(1) the number of tires received at the facility;(2) the number of tires that came from DEQ approved community-wide cleanup events;(3) the number of tires that came from dumps on DEQ's priority cleanup list;(4) a summary of the destinations and intended uses of outgoing shipments of processed tire materials, including the number of tons of processed tire materials per market category; and(5) the amount of processed tires provided to entities identified in OAC 252:515-21-71(a)(3) that were not in ready-to-use condition when received by such entities.(b) Quarterly reports. No later than the 10th day following each calendar quarter, each facility shall submit a report to DEQ that documents statewide collection efforts for the previous quarter.(c) Annual reports. No later than the 10th day of January each year, each facility shall submit a report to DEQ that its scales have been tested and certified as accurate by the Oklahoma Department of Agriculture, Food and Forestry.
