SECTION 310:664-5-7. Program application

- (a) An institution which desires to sponsor a preparedness program shall apply on the Department form.(b) No preparedness program shall be operated, and no participant shall be solicited or enrolled, until the Department has approved the program.(c) The application shall include, but is not limited to, the following:(1) A course syllabus that outlines course content including total hours for implementation, time per session, number of sessions, and the calendar dates for each session;(2) Instructor qualification information;(3) A copy of the certificate to be issued to participants at completion of program;(4) Location of the classroom;(5) Program objectives;(6) A description of the learning environment including, but not limited to, the following:(A) Location of and accessibility to the site;(B) Adequacy of lighting;(C) Accommodations for handicapped;(D) Accessibility, safety, and sanitation of personal conveniences;(E) Controlled system to heat and cool air; and(F) Number of participants the classroom can accommodate.(7) Letter(s) of agreement for instructional use of the facility signed by the facility administrator and the coordinating representative of the applicant.
