SECTION 365:10-5-180. Public disclosure

Latest version.

(a) The disclosure required by Section 6055(B)(2)(f) of Title 36 shall be in writing on a document no smaller than five inches by seven inches (5" X 7"), in type no smaller than twelve point and posted in easy view of the public.
(b) When any referral is made and disclosure is required pursuant to the Oklahoma Health Care Freedom of Choice Act, that disclosure shall be in writing within a document with print no smaller than eight point delivered to the insured prior to point of service.
