- (a) A certified program that uses hand written records shall have written policy and procedures for correcting errors on hand written record material by lining through, initialing the error, and inserting the correct material either above the error or at the end of the entry. Further, the policy and procedures shall forbid the use of "white-out" or any action which obliterates the error.(b) Each client record entry shall be legible, dated, and signed by the staff member or volunteer making the entry.(c) Copies of all service documentation including assessments, exit interviews and reports shall be kept in the batterer's file.(d) Compliance with 75:25-3-3 shall be determined by on-site observation, client records, policy and procedures and any other supporting program documentation.
